Figlia di Eduardo Cruz (commerciante) e Encarna Sánchez (parrucchiera). Iniziò la sua carriera artistica in televisione. Il debutto cinematografico avvenne con il film Prosciutto, prosciutto (Bigas Luna, 1992). Tra un film e l'altro, ha continuato a studiare teatro nella scuola di arte drammatica di Cristina Rota, studio che continuò anche nelle città di New York e Londra. Il film che le ha dato il successo e la fama è stato Tutto su mia madre (Pedro Almodóvar, 1999).

Lei stessa fa risalire il suo interesse per una vita artistica ad un concerto del musicista Prince a Madrid.

L'interpretazione di Raimunda nel film Volver, le è valsa la candidatura all'Oscar per la categoria Miglior attrice protagonista nell'edizione 2007 degli Academy Awards.

Ha una sorella e un fratello: Mónica, attrice e ballerina, ed Eduardo, musicista. Ha avuto una relazione sentimentale con il compositore Nacho Cano del gruppo musicale spagnolo Mecano e con l'attore Tom Cruise, dal quale si è separata all'inizio del 2004. Recentemente ha avuto una relazione con l'attore Matthew McConaughey conosciuto sul set di Sahara.

Dall'autunno 2006 è testimonial delle linee di gioielli BrosWay (Italia).

Il 2 marzo 2007 è stata ospite della 57ª edizione del Festival di Sanremo.

Pratica la filosofia buddista ed è vegetariana.
